The Forms panel is the place to input and review additional data about your slides

Forms comprise of question/fields that you define.

Forms are completely customizable with many different datafields including:

• Numerical
• Freetext
• Checkbox
• Multiple choice
• Dropdown menu
• Percentage
• Date

Forms can be assigned at both the slide level, and at the folder / directory level so that you can have case-level and project-level data.

Please note forms need to be defined / created in PMA.core. Please ask your administrator if you don't have the form(s) you need.
